How Electrotherapy Helps Neuropathy Sufferers

Electrotherapy involves the use of electrical energy in treatment of various diseases. Electrical therapy includes various modalities for a variety of diseases, however in patients with neuropathies, the electrotherapies mainly used are electrical muscle stimulation and electroanalgesia.

The form of electrotherapy, called electroanalgesia specifically deals with use of electricity to provide relief from pain. The nerves carry the pain signals as electrical signals, and electrotherapy provides symptomatic relief by interfering with these electrical currents of the nerve fibers. There are numerous techniques for providing electroanalgesia and the ones most commonly used in treatment of pain associated with peripheral neuropathy are peripheral nerve stimulation, percutaneous electrical nerve stimulation (PENS) and transcutaneous electrical nerve stimulation (TENS). The techniques that can be used in pains associated with neuropathies, like diabetic neuropathy, are Peripheral Nerve Stimulation and TENS. Another subtype of electrotherapy, called electrical muscle stimulation is widely used for various indications. In neuropathy, it is used in rehabilitation if the peripheral neuropathy has caused loss of motor function, for e.g. as in Guillain-Barre Syndrome. The paralyzed muscles are stimulated electrically at regular intervals to prevent muscle atrophy while the nerve slowly heals. Electrical muscle stimulation during recovery of nerve function helps in faster improvement of strength and muscle control.

Patients with autonomic neuropathy may have urinary and fecal incontinence. Electrotherapy can be tried as a therapeutic option in such patients. Often the treatment provides at least some beneficial effect and may even cause complete continence. Electrotherapy can also speed up the wound healing. This is especially important in diabetic patients, where poor wound healing is a troublesome complication of the long-standing diabetes mellitus. Although more research is still needed, the current research data supports the efficacy of the electrotherapy in wound healing. It is postulated that electrotherapy exerts its beneficial effects on wound healing mainly by enhancing the microcirculation and protein synthesis in the area to which it is applied.

A true story.

For the last several years I have been watching stories about nursing home neglect and abuse. I am older and am curious about what lies ahead for me.

It really hit home when the Mother of a lifetime friend of mine developed Alzheimer's. Her husband is still alive and they are both in their mid 80s. She was eating things she wasn't supposed to eat or was taking too many meds, etc. The bottom line, it was no longer practical for her to stay in the home. It was a very hard decision, but her husband finally agreed it was time to find a place that would care for her.

They live in a small town, so the choices were limited. The choices were a nursing home that cost just under $4,000 a month versus an Alzheimer's facility that cost even more. The nursing home was a lot closer, so they chose to take her there.

She's been there 8 months. The family suspects that there is some neglect going on. When they visit, she will still be in her gown or not dressed properly. Just little things, but they add up. The nursing home is telling them she needs to be in an Alzheimer's unit so she can have more individual attention. But at this point, they feel they can't afford it and there is a waiting list.

If already suffering from neuropathy symptoms, then along with control of blood glucose, good care of feet is must. One should daily see the feet thoroughly for any injury (which may also be just due to shoes). Always checking the shoes for any foreign material in it would be a good habit. I have PKD, Polycystic Kidney Disease, and in September of 2008 I started on peritoneal dialysis. In 2006 I was referred to Dr. Samuel Schorr, a Nephrologist here in Calgary where I currently live. I started with Haemodialysis in May 2008, I found the treatment to be effective, but time consuming, and felt I could do more for my own treatment. I decided to try peritoneal dialysis, it is a much gentler treatment, and can be done by myself at home. I had a catheter surgically inserted into my peritoneal cavity in the summer of 2008 at Foothills Hospital, and took training at the Col. Belcher Hospital (one of the last through there, there was moving schedules on all the notice boards, they tore the building down shortly after, it's dated from WW1, they had built the Sheldon Chumir Medical Centre to replace it, that is where I go now) at the end of August for peritoneal dialysis, followed by training for nocturnal peritoneal dialysis, the cycler.

I would start each day by draining the solution that had been in while I slept. I would then empty a 2 litre bag of Dianeal (1.5% or 2.5% sodium and water) solution into my peritoneal cavity, my stomach. That is good for 4 to 6 hours. I would take a 2 litre bag to work and do an exchange sitting at my desk during lunch, an exchange took about 40 minutes, and I could sit at my desk and work while doing this. I would do another exchange when I got home from work, clean my house, then do my last exchange before bed. This time I would use an Extraneal solution, 7.5%, that would last for 12 to 16 hours, while I slept. In the morning, repeat, no days off on Peritoneal Dialysis.

I only followed this routine for a couple of weeks, then I started on nocturnal peritoneal dialysis, the cycler! You must be very careful to maintain a sterile environment while on both types of treatment, I am normally a slob, but my apartment was very clean while doing this. I would scrub for at least one minute before doing an exchange, and I would wear a mask. When you swap bags you are exposing your stomach to the outside air, full of nasty things your tummy is never supposed to see, so you must be clean, and quick!
